  • Wat to fill in DS156

    I have two doubts in filling this form for my parents, kindly help.
    1)My parents mailing address is different from their current address. Due to some reason they dont receive mail at their current address which is the same as mentioned in their passport. What should I fill in the home address, the address in their passport or the mailing address. If I fill their current adress(the one in their passport), i am afraid the passports might be lost.
    So can i fill in their mailing address as their home address, will it be an issue, as the two addresses are different.
    2)If I fill their application form do i have to fill in question no.40, also or it is only if an attorney fills the form.

    Q1. - Current address in that case, since you don't have much choice anyway ...

    I am sorry but I am not going to find the form and read the #40 to reply to your question. If you'd posted the question I would've been able to make sense out of it. Hope someone else knows the question and will be able to reply.

      I am sorry,should have written in detail,the coloumn 40 in ds156, is the name address and contact details of the person who filled the ds156 form for the applicant. I read somewhere that this column is to be filled only if the application is filled by an attorney, since I am filling the application for my parents, do I need to fill in this column and sign it, or should I leave it blank saying that the application was filled by self. Thanks and appreciate your help.

        If your parents ...

        ... do not read/write in English and you're filling up the form for them, then you can write your name as the person who filled in the form.

        Then you'd write down your relationship to the person.

        If you're just filling the form to help them out or if your handwriting is better, then you don't have to worry about it. You can still put your name as the person filling the form. It's not going to cause any harm to you or your parents.
