Question on the departure form in the flight- Disembarkation card

HI All

Can anyone help me in filling the form when departing back to india that's given in the flight....


Asking for Date of Arrival,
Flight number
Port of boarding - Is this the place from where the passenger was leaving from US
Port of departure from India - Is this the place from where the passenger left from india to come here..
Departure date from india

And also when they are asking for dutiable goods being imported , if there is nothing important like no electronics then whats the approximate that we have to right,
