B2 I-134 form

In the B2 Visitor Visa I-134 form, it is asking for the “If a lawfully admitted nonimmigrant, give Form I-94, Arrival Departure Number”.

I have three “I94” forms with me – starting from the very first one till the current one. Which number should I specify here?.

I have previously submitted visa petitions and affidavits for my parents 6 months before. Currently am applying visit visa for my Uncle and Aunt.
So in the sections ‘I have submitted visa petitions and affidavits for the following persons” should I specify my parents name there ?.

Please clarify on the above mentioned points. Thanks.
