Sorry, didn't mention. I am on EAD, and applied 485, i have case number like Axxxxx. Do i have to specify anywhere in I-134 suuport doc.

In question 1,
There are 4 options, a, b, c, d. You should find little space below d and write on your own that you are "Adjustment of Status" and write your A number there.
