134 Question Urgent Help Please

I have to send 134 for my parents. I have few doubts, I would really appreciate anyone's help.

Question 3 on 134: Name
My parents passport doet not contain Last name(Family name). It just has First Name and Middle Initial. Should I specify Last name or not in the family name. Or Should I specify Middle name in family name?

Question 9 on 134: Affidavit(s) of Supports

I have filed 485 and I think(not sure was long back) I submitted 134 for my spouse and kid. Do I need to specify that or this question is only for 134 submitted for visa?

Question 10? Visa petritions
Is this question also related to 485 applications that I have filed for my family? Do I need to specify about my spouse and kids 485 filing in this?

LNU = Last Name Unknown.

10. No. This is for immigration petitions you might have filed. Not I-485 dependents when you are employment based GC applicant.
