Need help on the letter format for medical reasons

My wife is pregnant our OB/GYN has issued a letter saying she would need help for medical reasons and asked the in letter to issue visa for her parents(or some one who can help). I spoke with the consulate today and they asked me to fax the letter for consideration, can some post a kind of sample cover letter if you have done earlier in this kind of situation any help will be highly appreciated, thanks in advance
