Reg DS156 Form

Hi,

Iam sponsoring my parents for visitor visa.My father didnt mention his Surname in passport and hence it is blank.When we book for an appointment online,DS-156 Form has the Surname field as mandatory.

Can you please let me know what has to be filled in that??

Put the given name as the surname and put given name FNU.
