Doubt in DS 156

I was filling the DS 156 form for my father .. I have a doubt regarding Question no . 18
Spouse's Full Name
(Even if divorced or separated. Include maiden name.)

Do i need to enter my mother name and including her family name before marriage?

plz let me know..

Yes, that is right. put both her current name and meiden name(before marriage name) if it's different from her current.

regarding question no 40

In DS 156 Form the question no 40 asks if the application was prepared by someone on behalf. I have prepared the DS forms for my mother. But I will put my fathers name anyhow. So will there be any problem if I put yes and give details ?
Will it create any negative impression for the visa officer?

The questions is to identify who’s filed the form and is the information correct or not, taking the conscience of the person for whom the form is filled.
It’s absolutely fine if you are writing your father’s name, make sure he is kept in loop about the same and is also aware of the details entered on your mom’s form.
