Urgent! Help required for documents to be prepared by the visitor.

Hi,
In Immihelp ,its mentioned that visitors can get a Property estimated from a
Chartered Accountant.Is there any format for such document?
How about the Affidavit prepared by visitors?Should it be on a Stamp paper if yes how much amount of stamp?
