? about filling form at the consulate

Hi,

My mother had appeared for the interview at chennai consulate in nov and her visitor visa got rejected. She was mentioning about the form that they gave everyone to fill out inside the consulate. Is anyone aware of such form and what is it about. I really appriciate your help.

If yes, you can someone relative in India to look it up and tell you what it is.

NO she doesn't have that form with her. They collected that form there it self.

Yes .. and consulates they are given another form inside which has question about the type of entry you are seeking and the language you prefere for the interview. This is more important at Channai consulate as persons appering there are multi-langual.
