Doctor visiting for observership

hello I am a doctor practising in Mumbai since 5 yrs. at a reputed hospital
Age-32
married
monthly income -1.20 lakh
wife employed working

i am visiting the US for obsevership at two hospitals . One month each. So total 2months.
i have received the invitation letter.
One is in NY and other is in OH.

my query==

1) in visa DS 160 form under section” Adress where you will stay in US” there is option to mention only one address of stay.however if my itinerary involves two separate places of stay how and where do we mention…?
DO we need to mention ALL adresses during our stay in USA ?

2) can I apply when my date of exit is not confirm
3) do we need confirm air tickets and hotel reservation at the time of visa interview?
