DS-160 surname

Hi!!
Need help
My daughter's passport has following
Name : Sxxxx Singh Surname : blank...so when i filled last Ds 160 i put in Surname: Sxxxx singh Name : FNU
However when i recd her visa it read Surname: Singh ;Name sxxx
Now i am applying for renewal ..what should i fill
surname: Singh Name : Sxxxx as per old visa
or
as per Old DS-160 i.e Surname : Sxxxx Singh Name: FNU
Pl. help

Did she used that visa for travelling to USA?

What are you renewing ? Passport or Visa ?

Thanx for ur prompt reply !!!
1 She did travel on that visa(b1/b2
2. current visa expiring so want to renew her bq/b2 visa
Her passport already renewed with no change in name ( same as old passport ie No Surname)
