How many times can I FIll Out a DS 1600

Hello everyone This is the situation, I filled out the entire form and signed it, and when I tried to enter the part to start a family procedure, my computer restarted. And when I tried again, it no longer gave me the option of the family.

I searched the internet and they recommended that I start a new one, but the same thing happened
