Maiden Name on DS-156

hello all,

1. in question 18, my mom is a widow , should we mention my father's name as in the passport , because passport just contains only first name of my father as spouse's name????

2. What is the maiden name that we should include in question 18 apart from spouse's name????

3. Business phone , can my mom give her office phone number in india ????/

I prepared all the documents using the good help provided by this website. But then there are few places which is slightly tricky and hence wanted to make sure from other's experiences on filling the same. Also only when we read other's experiences with the visa forms , I came to know so many were rejected just by grammatical errors or form filling errors like in the invitation letter instead of visitng daughter if the sponsor states that the parents are staying and in the interview they say STAY INSTEAD OF VISIT THEN ITS REJECTED , JUST ONE WORD CHANGES THE ENTIRE PROCESS.

So just wanted to make sure , please be cooperative and help forum hubbers as you always been to me especially divya, karthik , srikar and IMMI.

Please let me know whAT DO U MEAN BY MAIDEN NAME IN QUESTION 14 OF DS-156. ALSO ONE MORE QUERY IS IT OKAY IF WE PROVIDE MORE AMOUNT IN BANK DEPOSIT THAN THE ONE WE STATE IN i-134 AFFIDAVIT???????

PLEASE REPLY SO THAT I CAN SEND THE DOCUMENTS BY TOMORROW.

Apeksha, as much as I can understand how much u r worryin on ur mom's interview....looking at the refusal stories here..., trying to do the paper work with worried mind will only increase doubts n qns...on even the simplest points.

I mention again, tht I am writing down the answers for u...assuming myself from ur end. I hope u try duin it urself next time. Take a minute to think and u will have ur answer.

Showing more bank balance than u mentioned is not a prob! Just make sure u dont' show way less than wht u mentioned.

Giving ur mom's office ph no is not an issue either. Usually this is never used in normal circumstances, as they decide the visa status then and thr at the interview.

Maiden name is ur moms' last name before her wedding. If there is any paperwork tht she is carryin which uses/has her maiden name on it, then she better mention. Also if she changed her name or if she used another name on some other documents, u have a clause whr u can mention those names. I donot know which item no of the form..but go thru it..and u will find a qn whr it asks to mention maiden or other names tht the applicant used. If she has used none other than one name, then just say NONE.

what exactly is maiden name , am still not able to understand. Do not shout at me. Because in that question 14. , it says spouse name and in the braces says include maiden name .
What does that mean????

Men donot have maiden names as they donot change their last names after their wedding.

Nywys, just write down ur father's full name with his last name in tht space. And it is not item 14, but 18.
