Visitor visa for my wife

Hi, I have some questions regarding getting a visitor visa for my wife. To give more background about us. I am frequent traveller to US and I have a 10 Yr multiple entry Business Visa as of now. . I am planning to take my wife along with me during my next visit. My wife is working in IT industry. What documents will be required for us to get her visa. Should we mention that she is coming along with me and I will be responsible for all her expenses.

Any help in this regard is welcome.
