hdfc fee for myself and spouse...both need to pay?

Hello friends

I am applying as a principal applicant for tourist visa

In section asking persons accompanying you i have written my wife name

do i have to fill whole new ds-160 form again for my wife or just 1 form i have to fill by my name and in person accompanying i write her name?

Also do i have to pay 2 seperate fees for each of us?
