Entry on visitor visa after 3 yrs of getting the visa stamped

I am going to US for the first time on my visa stamped 3 years earlier. What all documents do I need to carry with me. Also is there anything extra that i need to do because I have not used my visa for three years and this is the first time I am going to use it ?

You can visit USA even on the last date of the expiry of your visitor visa and so you do not worry on that issue.
You need to carry with you a to and fro air ticket, contact address and phone number where you will be staying to fill I 94 form and a health insurance cover.
