I-94 filling Question

Hello,
In the I-94 form for entries related to

city where you boarded
and
airline and flight number

what should one fill? Is it the starting city/flight number or the transit city/flight number?

For flight from Delhi to SFO via frankfurt by lufthansa does one fill
Delhi or Frankfurt as "city where you boarded"
and
airline and flight number as from DEl-FRankfurt or Frankfurt to SFO?, where layover time in Frankfurt is ~ 3hrs.
