Usa tourist visa query?

Hello,

In 2009 , my dad applied for a tourist visa and the main purpose was to visit his newly born nephews however his application was declined because he was a widower. His wife, or my mom died in 2005. My dad was an aged person and he also had a injury in his right leg so he could not go there alone I was accompanying with him for the trip and then I had to return back to my home country after two weeks.

my dad was a main applicant and my name was written as an accompanied applicant.

However, I would like to apply for a tourist visa this time on my own. I have visited 5 countries before on my own. I run my own business and im also a student at a university. I would like to go for tourism in the winter holidays.

my question is, have you ever been refused a visa to USA before? Should I say YES or NO to this question on the application form?

Did you actually apply for visa yourself?

If not, how did you even accompany him to the interview?

Let me describe the whole scenario:

My dad wrote my name "Adeel Khan (son)" on his main application form to the question 34:
"Names and relationships of Persons traveling with you".

I also filled in the main application form and wrote "Siddiq Khan(Father)" for the same question on my application form.

I also paid visa fees . My father also paid his visa fees.

So, yes, we had filled in the forms separately for ourselves but we went to the interview booth together. This is what happened at the time of interview.

An Afro American lady asked my dad: I was not interviewed at all.

Q1: Who is this?
A: my youngest son

Q2: What is the purpose of your trip?
A: to visit newly born grand kids

Q3: Did you ever travel to the USA before?
A: in 2000 and 2004.

Q4: How long did you stay each time?
A: 6 months

Q5: How long will you stay this time?

A: 6 months (as written in the form)

Q6: How long will you stay this time?(asked this question again)
A: 6 months

Q7: When did your wife die?
A: March 2005 (as late written on the form next to her name)...she picked on it....

Q8: When did your wife die?
A: March 2005 (asked again)

Q9: When did your wife die?
A: March 2005 (asked again)

Now she had handed over the paper and backed off from the window. The translator, pakistani girl told my dad they could not issue him visa according to American law.

Kindly tell me , so my previous application was considered refused based on the above scenario. if I want to reapply again this time as myself, will I have to say YES to my original question?

PS; BTW, Visa officers no longer put application recieved stamps on passports here at Islamabad and Karachi embassies. So , my dad and I did not have any stamps on our passports.

previously i paid my visa fees , filled in my visa application but I was not a main applicant my dad was. he was interviewed, not me. he was declined so i was declined automatically....
