Visitor's visa- Need advise on insurance(s)

Hi all, Can somebody enlighten me on the difference between travel visa and medical insurance?
1.Is it mandatory to have the insurance to get into US.
2.Should one buy the policies from an US Co.or even the Indian ones are as good? What would be the cost difference?
3. Any reccommendation on Indian co. selling the policies?
The age of the applicant(s) are 45yrs and 13 yrs, and are both healthy and would be at US for approax 1 month and 20 days.

Suggestions and advises are welcome.
