H4 Visa pending for more than 180 days

Hi all,
I have filed H1 ext and H4 ext separately as my family joined after i i submit my H1B extension . I got approval on my H1B and my H4 Ext is still pending. it has been 180 days that we filed Extension for my family . Can my family stay until 240 days ? What are all could be reasons for pending ?
Note :- i have filed for extension before I-94 Expires .

Yes, 240 days.
