VO gave me a Voluntary Declaration Form and told me to write my Roles and Responsibilities and told me to sign and they gave me Blue Slip and told me to email the docs by my employer .
They told me to mail Project Description in detailed and docs submitted to the USCIS.
If any one got the Voluntary Declaration Form then please let me know.
Also i wanted to knw why was "Voluntary Declaration Form" was given .
