I just received a White form (not sure if that is what is known as White Slip) which had instructions on what I need to send (documents, questionnaire, etc). Sent all documents on Jan. 5, 2016. No updates yet. The status on CEAC website was last updated to January 28, 2016 on February 4 or 5 I think.
Me and my wife(H4) went together for the interview. Both Passports were taken. The Consulate officer did not tell me anything regarding 221g. She took the passports and asked me to go to another different window (that is, not to another consulate). The person in that window, told me about the 221g and handed me this document.
Waiting since then!
