Infant Visa for India

Hi All,
I am about to apply a visitor visa for my daughter ( 1 year old) who is a US citizen and filled out the online app visa the bls-india website. Our jurisdiction is Atlanta, but on the BLS site it doesn't give you a fee for any visitor visa when it comes to Atlanta. I saw somewhere else on a general note about fees for multiple entry for a Indian visa is $153 + 4.70 = 157.70 for a 10 year visa. Is this correct?
And how do we sign this application since the applicant is a infant?
And the required documents are birth certificate and parent's passport? I saw in one place on the BLS site that the visa application should also be attested..is this correct?
My wife is a US citizen and she changed her name, but she has the a valid indian visa with her old name in the old US passport, do we need to change the name on the visa to match the new name on the new US passport?

I have tried calling Indian Embassy and BLS-India here in the US , but it goes to voice and I have sent out a few emails as well but NO response from these people.

If anyone has any idea about the above queries, that will be great.

Hi,
I got entry visa for my Kid, if anybody sending documents use fedex or ups, I had used USPS with return shipping with same USPS one day express which needs 20$ postage but once passport came back from consulate, BLS told me that there is no return envelop included so they used the postage I had included and I have to travel to SFO to collect the passport.
After that they dont ans to any calls so u will loose your return shipping money, so try using fedex with only lable.
