ppapp1 form? BLS checklist for No-1 form - Passport reissue after 10 years

Is this No-1 form different from what I fill online and download to complete?

there is a ppapp1 form here which is called the NO-1 form

The checklist here from BLS asks for a NO-1 form...

The reason for doubt is that hte ppapp1 pdf form asks for 3.5 cm pictures where as the online form and other forms mentioned asks for 2 inch by 2 inch pictures. Some one please clarify if this ppapp1 form is needed at all?
Whats the correct form to fill? Any one recently renewed their passport through BLS can help

You just need the online form. Photo should be 2 x 2.

Earlier, it used to be called No. 1 form. That is why they are referring like that.
