PIO for new born

Hello Friends,

I need to apply for my new born's PIO. She is born in USA while we parents are Indian Citizens.
What all docs would be required if I plan to apply in person at the DC embassy? If someone can pin point the exact docs here for our case?
Can I pay the fees by card or do I need to carry cash?
Do I need to make an appointment before I go or walk ins are also allowed?
please guide.
