Change of Name(Passport)

Dear Sir/Madam,
I am a citizen of India and presently a student in US. I want to
change my name in the passport. I have one query:

What address should I write in the (i) Notary Affidavit and
(ii)advertisement to be given in Newspapers, i.e. my permanent
address of India (as in my passport) or my address of US. If its
permanent address of India, then should I give the Indian address in
advertisement to be given in Newpaper of US? or should I give both the
addresses?
