OCI Application Form for third child

To whom it may concern,

I need help with the form, which I need to fill out. I have seen the form online, I have three children but it only allows me to put my two children's names. Where and How can I apply for my third child. Do I need to fill a different form. Also on the form its asking for Visible Mark please explain what this means.

Can you kindly email me the form I need to fill out and explain the question above as soon as possible. Also please advise me the fees.
