documents for OCI application

Hi, I am in the process of getting citisenship of US. Hence i will be applying for the OCI card once i gte my US passport. I was reading about the process of application and the documents to be submitted. One of the listed document was an affidavit. What kind of affidavit do we need to provide. Can someone please explain this in detail. It will be of great help.
