Questions for passport renewal:

1. Where and how do I get the documents notarized if any?

2. What addresses should I enter in the application form?

3. What if the Given Name(s) is empty in my current passport?

4. Do I have to create an affidavit for appearance change of around 10 yrs? i.e., when the passport was issued?

5. What are the visa documents to be given for Proof of Status if there is no green card or EAD documents applicable?

Thank you very much for replying. I got the answer to point no. 2. The answer to point no. 4 is probably 'yes' but not sure. For the other points I am still trying to find the answers... I have emphasized on what I wanted to ask as below:

1. Where and how do I get the documents notarized if any?
[Me] I asked this question since I do not know who and where is the notary to get the documents signed. I live in Des Moines, Iowa and I am not sure where would I find one.

2. What addresses should I enter in the application form?

3. What if the Given Name(s) is empty in my current passport?
[Me] Would I have to explicitly go for a name change right now since my passport expiration is nearing which is on August 19th this year, I thought I would give preference to passport renewal and then go to the name change. I am not sure if I am assuming the right thing here. Do you think I can do both of them comfortably within the time left? Will changing the name be a different process of both name change and renewal can be done together?

4. Do I have to create an affidavit for appearance change of around 10 yrs? i.e., when the passport was issued?
[Me] I think I have to do an affidavit but not sure that do I really need to. Most probably 'yes'. So I wanted to know where and how would I do this affidavit. I am not sure about how and where to go and do this.

5. What are the visa documents to be given for Proof of Status if there is no green card or EAD documents applicable?
[Me] I have the visa documents but I wanted to know which of those documents are required. My guess is the documents would be I-797, I-797A, I797B and I-94. Is this right? If no, what are the actual documents required?

4. If you had just normal age progression in 10 years, it is not required.
5. Copies of H1 approval notice and I-94.
