Indian Passport

Hello,

My Passport has only first name and no surname. At times I face problems because of this. I want to add my family name to last name column. I am based out of california and I am on H4 Visa. How should I go about adding a surname in my passport? it will be great if you could assist me with this.

I went through this prior to mailing you guys, could you just let me know which category should I apply for? is it change of name complete(as I am not changing my name here) or splitting name(there is no split happening)
