H1 to H4 transfer requirements, help!

I am on H1 right now. Not working for the past 2 months. I want to go back to H4 visa. Do anybody know the formalities? Do I need a lawyer or can I file it myself? What all are the documents and forms required for this?

Check the correct boxes in the applications. Must attach a copy of I 94.

Ok, do I need to give last paystub too? Mine is not the latest, its 3 months old. What are the other documents I need to provide along with I-539?
