help to file a complaint WH-4

Hi Guys

I've not received my salary for last 4 months from my employer.
I am planning to file a WH-4 complaint against my employer

i talked to dept of labor Hour and wage department they didnt' know much about how can i file the complaint

My company is based in Virginia
and i am working at the client site in California

Can you please guide me that where can i sent my WH-4 complaint form
I need to take an action soon or i'll loose a lot of money here
