Adding spouse name in passport

I am planning to add my name in my wife's passport. I saw below link for the process to add spouse name. I have few doubts in this. Can you please clarify.

1. For adding spouse name in my wife's passport, do we need to send both of our original passports to embassy or
only my wife's original passport and notarized copy of my passport are enough.

2. If we have to send both of our original passports, will they add spouse name in both of our passports or only in my wife's passport.

3. Do we need to fill two application forms and two return mail envelopes if we need to add spouse name in both of our passports.

Thanks for the update. My question 1 is if we need to add spouse name only in my wife's passport. Do we need to send both our original passports?
