Process to extend B2 visa for Parents

Hii I have couple of questions regarding extension.

1. Do we have to fill the forms online or by hand?

2. Do we need i-134 form along with i-539?

3. We stay in San diego.Ca. What is the Mailing address to post the forms?

4. If the visa is not extended,does that mean the loss of $300? is there any refund process?
