Lost Passport with multi-entry B2 Visa while in US

My father lost his INDIAN passport while in US visiting us. His passport had multi entry B2 Visa stamped in. Need help on the next steps. Urgent !!!

He has to do everything all over again.

File police complain, get new passport, go home and get new visa.

What are the possible documents required for getting a new passport. At this moment he has his PAN Card, Driving License. We also have the Xerox Copy of the Passport[1st and last pages], ofcourse all the VFS documents however we do not have a copy of the I-94 or Visa stamped.

Please advice.
