While completing an online application, in the 'service desired' field, choose option 'New Passport in lieu of
Damaged/Lost Passport'.
Applications for duplicate passports are normally not accepted by mail except with prior approval of the Consular Officer.
And such approval is normally given when the persons lives far away from the Consulate. In these exceptional cases, the photograph
and signature of the applicant must be notarized on a separate sheet of paper with the notary's remark "Seen the person and signed in
my presence" which can be sent by mail along with the application.
A duplicate passport may not be issued for the second loss/damage of passport. Habitual losers of passports will be subject to
enquiry.
If you are a student or a trainee, a short validity passport valid for one year may be issued in lieu
of a lost/damaged passport based on the certificate issued by the concerned educational/training institution to
the effect that the individual is a bona fide student of that institution and that he/she is of Indian nationality
as per the academic record submitted by him/her at the time of admission to the institution. The short validity
passport needs to be renewed for full validity on receipt of confirmation from the original passport issuing
authority or on the expiry of three months from the date of making original reference, whichever is earlier.

- If you are married and wish to include spouse name in the passport, additional documents, except
your own passport which you can submit because it has been lost.

Four (4) copies of Personal Verification Form. This forms differs by the Consulate.

If the passport expired over 5 years ago, prepare six (6) copies.
- Photographs:
Total six (6) photographs.
Paste two (2) on the print out of the online passport application; and one (1) each on the four (4) copies of the
Personal Verification Form
- If the Indian passport has expired more than a certain period, attached a notarized affidavit.
- Affidavit for Embassy of India, Washington DC (when expired more than 1 year ago)
- Affidavit for Consulate General of India, San Francisco
(when expired more than 6 months ago)
- Affidavit for Consulate General of India, Houston (when expired more than 1 year ago)
- Affidavit for Consulate General of India, Chicago (in all cases)
- Consulate General of India, New York: The declaration at the end of the application form should be
notarized if the old passport had expired more than one year ago.
- If you are a student
- Current school/college/university ID
- Letter from school/college/university in the USA
- Proof of stay in the USA, from the date mentioned in the application by the applicant.
- Indian Photo Identification Card such as Driver's License, Ration Card, Voters Card,
PAN card, or any other document containing photo and indicating Indian nationality of the applicant.
- Birth Certificate.
- Matriculation/Higher Secondary/School Leaving Certificate or any other school records from India
or the USA.
- Notarized affidavit from two Indian passport holders, who have
valid resident status in the USA (they should not be short term visitors).
- Original Police Report filed with the Police about loss of passport.
- A letter on a plain paper addressed to the Consulate General of India, <CITY NAME>, explaining
the circumstances of the loss of passport.
This letter is also called an 'Incident Report'.
