If you have scheduled an appointment through VFS in India for US nonimmigrant visa, each applicant that is 15 to 55 years of age must also fill out DS-157 along with DS-156 and must sign it by hand.

When you take an appointment online with VFS USA, you will be asked to enter all the details of DS-157 form online if you are 15 to 55 years of age. After filling all the details, you need to print that DS-157 form, which is essentially same as regular DS-157 form. There is no bar code or HDFC receipt number.

In most other countries, DS-157 is required only for men between 16-45 years of age. However, it is required for all applicants age 16 and over who are from Cuba, Iran, Libya, North Korea, Sudan, Syria, China, Northern Cyprus, Russia, Somalia and Vietnam. Applicants who have a criminal conviction, are HIV positive, have a medical ineligibility, have been denied entry into or deported from the US must fill DS-157 regardless of age and gender.

A consular officer may ask any applicant to complete this form.

Please ensure that all fields are completed. If you have no response for a question, put "none" or "nil". Do not draw a line or write "NA" or "not applicable".

All applicants above the age of 14 must sign their application form. Where applicant is below 14, either parent can sign the application form.

Applicable if you have filled the form online while taking an appointment:
You may amend, at any time, your completed and printed application forms in pen if the details of your application change or you made a mistake. Do not complete and print another DS-157 Form. Amend your original forms instead.

All applicants are personally responsible and accountable for the answers on their application forms, regardless of who fills out the form.

Q. 3 - [Full Name (In Native Alphabet)] You must write your full name in your native alphabet such as Hindi, Gujarati, Tamil, Telugu. This could be Hindi script if you are an Indian.

Q. 12 - You need to provide your previous employment details if you are currently retired.
